4D Molecular's One-Shot Eye Therapy Delivers Two-Year Win

On Saturday, 4D Molecular Therapeutics Inc. (NASDAQ:FDMT), a late-stage biotechnology firm focused on disease-targeted therapeutics, shared two-year results from its PRISM Phase 2b trial of 4D-150 in a broad wet age-related macular degeneration population.

Key Efficacy Outcomes Through Two Years

The Phase 2b study evaluated a single intravitreal dose of 4D-150 across two dosage levels, 3E10 vg/eye and 1E10 vg/eye, enrolling 45 total patients.

The 3E10 vg/eye level was selected as the designated dose for the upcoming 4FRONT Phase 3 trials.

Within the broad study group, 30 patients received the 3E10 vg/eye dose, while 15 received the lower dose.

A subgroup of 15 recently diagnosed patients—those diagnosed within six months—also received the 3E10 vg/eye dose to align with the Phase 3 trial target population.

As of the May 18, 2026, data cutoff, treatment with 4D-150 demonstrated consistent maintenance of best corrected visual acuity and steady control of central subfield thickness.

Patients experienced a meaningful, long-term drop in treatment burden across two years, maintaining a clear dose response in favor of the Phase 3 dosage.

Substantial Reductions In Patient Treatment Burden

Overall, trial participants achieved a 78% reduction in overall treatment burden, averaging 2.7 supplemental injections compared to the projected 12.0 injections for on-label aflibercept 2 mg administered every eight weeks.

The recently diagnosed subgroup demonstrated an even greater 87% reduction in treatment burden, averaging 1.6 supplemental injections per patient.

Safety And Tolerability Profile

Safety data covering 71 patients across Phase 1/2a and 2b trials at the Phase 3 dose showed 4D-150 was well tolerated.

Only 2.8% of patients (two individuals) experienced mild, transient intraocular inflammation within the first 28 weeks post-dosing.

Beyond 28 weeks, no new cases of inflammation were observed with two to over four years of follow-up.

Furthermore, no cases of hypotony, endophthalmitis, vasculitis, retinal vasculitis, or choroidal effusions were recorded.

In February, 4D Molecular Therapeutics completed enrollment in the 4FRONT-1 Phase 3 trial of 4D-150 for wet age-related macular degeneration (wet AMD), with topline data expected in the first half of 2027.

In June, the company completed enrollment in 4FRONT-2, the second Phase 3 trial in the global program evaluating 4D-150. Topline data is expected in the second half of 2027.

On Monday, HC Wainwright & Co. analyst Matthew Caufield reiterated 4D Molecular with a Buy and maintains $36 price forecast.

FDMT Stock Price Activity: 4D Molecular Therapeutics shares were up 2.82% at $11.30 during premarket trading on Monday, according to Benzinga Pro data.
